Title: PROPOSED ORDINANCE DESIGNATION OF COOK COUNTY AS A HYBRID ENTITY FOR THE PURPOSES OF COMPLIANCE WITH THE HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT WHEREAS, the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its Rules (codified in the Code of Federal Regulations at 45 CFR parts 160 and 164, Pub.
