HONORING CHIEF NURSING OFFICER OF COOK COUNTY DEPARTMENT OF PUBLIC HEALTH DENISE GILBERT ON HER RETIREMENT

WHEREAS, Denise Gilbert, Chief Nursing Officer of Cook County Department of Public Health and oversees seven grant supported programs that involve nurse home visits and case management for the High-Risk Infant Follow-up, Lead, Perinatal Hepatitis B and Genetics programs, along with Case Management and Direct Observation Therapy for TB services, and the Case management and provision of screening services for our Breast and Cervical Cancer and Hearing &amp; Vision programs in the schools; and

WHEREAS, Denise and her team are responsible for nursing readiness to respond to communicable disease outbreaks and other PH emergencies; and

WHEREAS, Denise began her career with Cook County Health in 2003 as Divisional Nursing Director of Fantus Health Center. She later had oversight of the West Cluster clinics that comprised seven ambulatory health care facilities; and
