SummaryThis increase and first of two (2), two (2) year renewal options will allow the Department of Risk Management to continue to receive Employer Sponsored Medical Benefits.
Cook County offers HMO and PPO health plan options. Premium and premium equivalent rates are reviewed annually based on enrollment and utilization. The Director of Risk Management remains authorized to execute the annual documents required to administer these plans, including Benefit Program Applications, Stop Loss Agreements, and HIPAA Plan Sponsor Authorizations to support HMO and PPO plans. Employee contributions are collected through payroll deductions, with the remaining cost supported by annual appropriations.
