Summary: This increase and first of two (2), two (2) year renewal options will allow the Department of Risk Management to continue to receive Employer-Sponsored Vision Care Benefits.
Cook County offers a vision plan to all eligible employees and dependents; employees do not contribute through payroll to the cost of the plan. The plan expenses are covered through annual appropriations.
This contract was awarded through a publicly advertised Request for Proposals (RFP) process in accordance with Cook County Procurement Code. Davis Vision, Inc. was selected based on established evaluation criteria.
