Health Care Service Corporation is commonly known as Blue Cross Blue Shield of Illinois. Blue Cross Blue Shield of Illinois provides health care benefits (PPO and HMO) to eligible employees and their dependents. This contract was originally awarded as a result of a Request for Proposals (RFP) in accordance with the Cook County Procurement Code. It is a two-year contract with two (2) one (1) year renewal options; this amendment represents the second and final option for renewal.
