Summary: This amendment to contract #12-18-121 with EyeMed Vision Care, Inc. and First American Administrators, Inc. is for continuation of services for employee vision insurance benefits for a period of one year. EyeMed provides vision benefits to eligible employees and their dependents. There is no employee contribution for this coverage and the plan is administered in accordance with collectively bargained plan designs.
This contract was awarded through a Request for Proposal (RFP) procedures in accordance with Cook County Procurement Code. EyeMed was awarded based on established evaluation criteria.
