Summary: FYI & Associates, Inc. provides janitorial services for the leased office space at 1644 West Walnut Street, Chicago, occupied by the Circuit Court’s Adult Probation Department. This request is to extend the vendor’s current Cook County contract No. 12-53-037 for three months, through 8/31/2016, and increase the contract amount by $4,782.00
The requested extension and increase would allow a continuation and transition of services while the new contract is both completed and implemented.
